Welcome to the Partnership page for Child Welfare-involved youth and their caregivers. We understand the challenges of navigating child welfare services and are here to provide support, resources, and guidance to ensure that youth and caregivers receive the best care possible. ​​​

Documentation​

To protect the confidentiality of the youth in your home, Partnership must understand who the caregiver is to whom we have the authority to speak regarding the care and support of the child welfare-involved youth. Partnership may request documentation from you to ensure this. Please contact your county representative for assistance if you do not have any of these documents. Examples of the type of documents requested include: ​​

  • Agency Foster Parent Agreement 
  • County Placement Agreement 
  • Agency Consent for Health Care Treatment of a Minor  
  • Authorization for Medical Treatment 
  • Court Placement Documentation 
  • Adoption Order 
  • Birth Certificate (adopted youth) ​​

If you are unable to send us the documents using the methods listed above, please contact your county representative, who can email them to us directly at eunit@partnershiphp.org​. Please note that at this time, we can only accept emailed documents from county email addresses.  

Key Partnership Services for Child Welfare-Involved Youth​

We are committed to assisting youth in the child welfare system. Partnership offers various services to help young people achieve better health outcomes, stability, and future success. 

  • CalAIM (California Advancing and Innovating Medi-Cal) 
    CalAIM is a statewide initiative designed to improve care and outcomes for Medi-Cal members. It emphasizes a whole-person approach and incorporates new initiatives that address social determinants of health.

  • Enhanced Care Management (ECM) 
    ECM provides individualized support to Medi-Cal members with complex needs, ensuring they receive the appropriate care and resources they need to thrive.

  • Community Supports (CS)
    These services address non-medical needs, such as housing support, food assistance, and more, to improve overall well-being and stability.

  • Transportation Services Department
    The Transportation Services Department coordinates non-emergency medical transportation (NEMT), non-medical transportation (NMT), gas mileage reimbursement, and other travel-related services for our child welfare-involved members. It is our responsibility to get them to their medically necessary, Medi-Cal covered services using the least costly method of transportation that meets their needs.

The Family Urgent Response System (FURS) – A F K Y
FURS is a Free 24/7/365 Hotline for current or former foster youth and their caregivers to call and get immediate help for any big or small issues you may be having. Visit the FURS Website

CYC empowers youth in foster care to take leadership roles in advocating for changes in foster care policy and practice. Visit the CYC Website 

Foster Youth Education Resource Hub – A F K Y
The California Foster Youth Education Resource Hub provides information and resources pertaining to foster youth and the interplay of education and child welfare systems. National Foster Parent Association (NFPA)  - F
The NFPA provides training, advocacy, and support for foster parents nationwide. Visit the NFPA Website 

Kinship Support Services Program – A K
Kinship Support Services Program (KSSP) provides non-financial support services to relative caregivers and the children placed in their homes. This program supports caregivers who have children who are dependents of the juvenile court as well as children who are not court-dependent children. KSSP also provides post permanency services to relative caregivers who have become the legal guardian or adoptive parent of a formerly dependent child. Visit the Kinship Support Services Program Website

Family Focused Treatment Association (FFTA) – A F K

Support for foster/resource parents with education on roles and resources to help in successful placements. Visit the Adopt US Kids Website

Adoption Support Alliance - A
A resource with a compilation of websites, books, blogs, and organizations to help navigate the adoption journey. Visit the Adoption Support Alliance Website

Adopt US Kids – Adoptive Families - A
Support for parents who adopt from foster care.  Subsidies, services, and training are available to help during the adoption process and throughout families' journey. Visit the Adopt US Kids Website

Adoptive Families - A
A resource that provides education and support for adoptive parents. It is the how-to-adopt and adoption parenting network. Visit the Adoptive Families Website

Foster Care Ombudsman – F Y
The Foster Care Ombudsman is a helpful resource for addressing issues related to foster care placements, complaints, and concerns. They are available to assist foster youth, resource families, and foster care providers with issues related to care, safety, and rights.
