Equity and Practice Transformation

In July 2023, the Department of Health Care Services (DHCS) announced the Equity and Practice Transformation (EPT) Payments Program.

The goals of this initiative are focused on:

• Advancing health equity
• Reducing COVID-19 driven care disparities
  
• Investing in upstream care models and partnerships to address health and wellness
• Funding practice transformation aligned with value-based payment models to allow Medi-Cal providers to better serve the Medi-Cal population
  

There are three pathways to the EPT Payments Program:

• Initial Planning Incentives Payments (IPIP)
• Provider Directed Payment Program (PDPP)
• Statewide Learning Collaborative (SLC)
