Community Supports

​The Department of HealthCare Services (DHCS) implemented the Community Supports (CS) services as part of the CalAIM initiative designed to address social drivers of health (factors in people's lives that influence their health). 

These services are provided as cost-effective alternatives to, or in-lieu of, traditional medical services or settings such as a hospital, skilled nursing facility admission, or a discharge delay. The provision of CS is voluntary for managed care plans and they are optional for beneficiaries.

  • Housing Transition Navigation Services
  • Housing Deposits
  • Housing Tenancy and Sustaining Services
  • Short-Term Post Hospitalization Housing
  • Recuperative Care (Medical Respite)
  • Respite Services
  • Personal Care and Homemaker Services
  • Medically Tailored Meals or Medically Supportive Food
  • Day Habilitation Programs (Butte, Colusa, Glenn, Placer, Sutter, Tehama, Yuba)
