SDH Local Innovation Fund Grants
In our effort to support the communities we serve address the social determinants of health, Partnership HealthPlan of California (PHC) has teamed up with 11 community organizations to remove the barriers preventing individuals from accessing quality care. In total over $2 million is being awarded as part of PHC's Social Determinants of Health initiative.
PHC has allocated two levels of funding. Implementation Grants allow for a maximum of $500,000 over a two year period and are focused on helping local organizations implement an intervention enhancing the effectiveness of the health system. It is the goal of this grant to fund innovative programs looking outside the box to bring quality care to individuals impacted by social determinants of health. Planning Grants allow for a maximum of $50,000 over a one year period and are focused on empowering communities to develop a community collaborative to focus on social determinants of health and/or employ a feasibility study and determine a plan to create a SDH focused initiative.
