Partnership HealthPlan's Quality Improvement Program

The PCP Quality Improvement Program (QIP), designed in collaboration with PHC providers, offers substantial financial incentives and technical assistance to primary care providers so that they can make significant improvements.

Although the PCP Quality Improvement Program evaluates performance on PHC's Medi-Cal line of business, PHC encourages quality, cost efficient care for all of our members. Incentives are based on meeting specific performance thresholds in measures that address the following areas:

  • Prevention and screening (HEDIS®)
  • Chronic disease management – diabetes
  • Appropriate use of Resources
  • Primary Care Access and Operations
  • Patient Experience
  • Advanced Care Planning
  • Anticoagulation Program
  • Asthma/COPD management
  • Patient Centered Medical Home

View and track your clinical performance at: https://qip.partnershiphp.org!

To obtain access to your real-time data for the 2012-2013 clinical measures, monthly refreshed denominators and weekly refreshed numerators, you must listen to our Clinical measure eReports training webinar, which you can access by visiting our Technical Assistance Program page.
