For Medi-Cal Providers
Initial Health Assessment
All newly enrolled members need to be offered and provided access to an Initial Health Assessment (IHA). Members need to have an IHA within 120 days of enrollment. The IHA consists of a comprehensive health history, assessment of health education needs, physical assessment and specific evaluation, tests, immunizations, counseling, follow-up and treatments. Provision of the assessment or that of a comparable comprehensive assessment needs to be documented in the patient's medical chart.
In an effort to ensure patients have timely access to an IHA, PHC sends providers a list of members who are due an IHA. You should receive this list on a monthly basis.
Individual Health Education Behavioral Assessment - IHEBA ("Staying Healthy")
During their Initial Health Assessment, members need to complete an IHEBA in the appropriate age category for them. The IHEBA can help you identify risky health behavior in order to promote positive lifestyle changes.
For Medicare Providers
The Centers for Medicaid and Medicare Services (CMS) requires that all newly enrolled Medicare members must have a full health risk assessment or a good faith effort to complete one within 90 days of the enrollment date.
PHC sends members a health risk assessment survey to complete within 30 days of the enrollment date. If the member does not return a survey we follow up with at least three call attempts to complete the survey. PHC will send the primary care physician (PCP) a summary of the health risk assessment for their records. It's important that you review this information with your member during their next visit.
PHC is asking its contracted provider offices to also make attempts to contact and schedule an office visit with all new Medicare members. This office visit helps to establish care and for you to understand the members medical history and assess any specific needs.
