If you want to join PartnershipAdvantage (How to Enroll):
As a dual eligible member, you can change health plans at any time throughout the year, on a month to month basis. There are some important things you should know if you want enroll in PartnershipAdvantage:
- Please call our Marketing Department at (866) 249-9933. Our Marketing staff can give you information about PartnershipAdvantage, and connect you to community resources to help you make educated decisions regarding your Medicare benefits.
- If you would like to enroll in PartnershipAdvantage, you can do so by calling our Marketing Department and request an application from PartnershipAdvantage.
You can send us your application in a few ways:
- You can mail the application back in a pre-paid self-addressed envelope
- Fax us the application at (707) 863-4117
- Drop off the application in person at 360 Campus Lane, Suite 100, Fairfield, CA 94534
You don't have to disenroll from your current Part D or Medicare Advantage plan to join! By joining PartnershipAdvantage you will automatically be disenrolled from your old health plan, with no breaks in coverage!
We always strive to do our best for our members. If you have concerns, questions, or need to file a complaint, call our Member Services Department at (866) 264-3626. They are available to help you solve your problems. You can also speak to a Care Coordinator to help you get the Medicare services you need. You can reach our Marketing and Member Services representatives from 8am to 8pm 7 days a week. From March 2nd to November 14th, 2008, calls placed on weekends and holidays will be received by our answering service. Messages will be returned within one business day.
