Medi-Cal - Programs and Services:

INFORMATION AND REFERRAL SERVICES

The organizations listed below provide information and referrals to the many community resources that are available.

  1. Solano and Napa County Residents may call HelpLink at (800) 273-6222 for information and referrals. The HelpLink website is www.helplink-sf.org.
  2. Solano County residents may also call the County Health and Human Services t 1-800-400-6001 for information and referrals.
  3. Yolo County residents may call the Yolo Public Library at (530) 661-5982 for information and referral services.
  4. Yolo County senior citizens may also call the Woodland Senior Center at 530-661-5868 or the Davis Senior Center at (530) 757-5696.

These organizations provide referrals to hundreds of services that you may qualify for. Below are some examples of the many services that these referral organizations can refer you to:

  • Homeless Shelters
  • Food Banks
  • Child Care
  • Housing
  • Financial Assistance
  • Legal Referrals
  • Disability Services
  • Services for Seniors

Nutrition Services

PHC has a Nutrition Services Program for adults, children and teens. This program offers nutrition education and nutrition counseling. To get services:

A child from birth to 20 years old must:

  • Be short for age
  • Be underweight
  • Be overweight
  • Have very low blood iron
  • Have lead poisoning
  • Be very sensitive to some foods
  • Be at the first stage of diabetes

For adults aged 21 and older, you must:

  • Have heart disease
  • Have diabetes
  • Have HIV or AIDS
  • Be overweight
  • Have nutrition problems or caused by drug use

Have other problems such as cancer, renal disease, liver disease, gastrointestinal disease, or eating disorder, or very poor nutrition.

DOMESTIC VIOLENCE

Are you concerned about your relationship?
Are you ever afraid of your partner?
Does your partner tell you what to do and who to see?
Has your partner ever threatened to harm you?
Has your partner ever pushed or hit you, thrown things at you, or forced you to have sex?
Does your partner threaten to have you deported?

You are not alone.

Abuse happens in every culture, every country, and every age group, and it may be happening to you. If you are being abused, you may feel frightened, hurt, confused, disappointed, angry, ashamed, or hopeless. Your partner might make it worse by blaming you. But no one deserves to be abused or threatened. You cannot stop your partner's abuse, but you can find help and support for yourself.

What You Can Do

Talk with somebody you trust: a friend or relative, someone from your job or house of worship.

Put together an "emergency kit" of things you would really need if you had to leave suddenly, such as identification, medicine, keys, and money.

Call the National Domestic Violence Hotline at (800) 799-SAFE (799-7233) to find out about domestic violence shelters and programs in your area.

Call the police if you are in danger.

Remember that you are the expert about your own life. Don't let anyone talk you into doing something that's not right for you. There is no excuse for domestic violence.

TRANSPORTATION TO YOUR DOCTOR APPOINTMENTS

If you have problems getting to your doctors appointments because of transportation problems, call our Member Services Department. We have information about transportation programs that are available in the community.
