RIGHTS AND RESPONSIBLITY STATEMENT
You have a right to:
- Receive information about the Partnership HealthPlan of California (PHC), its services, its contracted practitioners and Providers, Members Rights and Responsibilities and all health care available to you. Receive a clear expanation of how to get these services and benefits.
- Be treated with respect and recognition of your dignity and right to privacy.
- Participate actively in decisions and understand treatment options regarding your medical care.
- Have confidential health records, except when disclosure is required by law or permitted in writing by you. With enough notice, you have the right to review your medical records with your doctor. You may request a copy of the plan's confidentiality policy by contacting Member Services.
- Be informed about PHC grievance process and understand how to use it. You will not lose your health care benefits if you file a complaint.
- Be informed if your medical providers are no longer available for any reason.
- Understand treatment options and participate with medical providers in decision making regarding your health care, to the extent permitted by law. You also have the right to refuse or discontinue treatment.
- Receive a second opinion regarding treatment options.
- Candid discussion of appropriate or medically necessary treatment options for your medical conditions, regardless of cost or benefit coverage, in a language that is best for you.
You have a responsibility to:
- Carefully read all of the information we send you after you are enrolled. This will help you understand how to use your health plan benefits and how to keep your out-of-pocket costs down.
- Follow the treatment plans your doctor develops for you. If you chose not to follow your doctor's plan, you must consider and accept the risks.
- Ask questions about your medical condition. Make sure you understand the explanations and instructions you are given.
- Make and keep medical appointments, notify your doctor ahead of time if you are not able to keep an appointment.
- Pay any premiums or copayments on time.
- Treat all PHC staff and health care providers respectfully and courteously.
- Provide, to the extent possible, information that PHC and its medial providers need in order to care for you.
- Tell your medical provider about your medical condition and any medications you are taking.
- Be on time to medical appointments.
