WHAT IS A PRIMARY CARE DOCTOR?

A primary care physician, also referred to as a PCP, is your personal doctor who will provide and arrange all of your medical health care needs. All PHC members, except under special circumstances are assigned to a PCP.

HOW DO I PICK A PCP?

Look through the PHC Provider Directory. Choose the doctor or medical group that you want. Some doctors and medical groups listed in the provider directory may not be accepting new members. To find out if a doctor or medical group is available, look at the section called "Enrollment Status" in the Provider Directory. You may also call the PHC Member Services Department for help.

Some doctors speak other languages in addition to English and may have staff who speak other languages. You can check to see what languages each doctor and medical group provides by looking at the section of the Provider Directory called "Languages."

WHAT IF I WANT TO CHANGE MY PCP?

If you would like to change your primary care physician or medical group, choose another provider from the PHC Provider Directory. Then you should call the PHC Member Services Department with your choice. Our staff will update your records and send you a new PHC Identification Card.

If you call the Member Services Department before the 15th of the month, you can start going to your new primary care physician on the first day of the next month. Calls received after the fifteenth of the month, will be effective on the first day of the second month form the date your request was received.

HOW DO I GET MY PRESCRIPTIONS FILLED?

If you have a prescription that needs to be filled, you should take it to one of the pharmacies on the pharmacy list that PHC sent you. If you do not have the copy we sent you, can get a copy of our contracted pharmacies from the section of this web site titled "provider directory" or you can call our Member Services Department.

HOW DO I GET A REFERRAL?

Your PCP will refer you to a specialist if needed. A referral to a specialist must be approved by your primary care doctor. If you have any problems getting a referral or problems with the referral process, you should call our Member Services Department.
